| Set 1: Find a date. You select the most convenient date(s) for the clinic. Step 2: Send out Consent Forms. Approximately one month prior to the scheduled clinic the Dream Team will send you a packet of information about the program. Included in this packet are Consent Forms to be sent home with each child.
Step 3: Return Consent Forms: The Consent Forms must be returned to the Dream Team two week prior to the clinic. The Dream Team will pick up the forms or a postage-paid return envelope will be provided for this purpose.
Step 4: Finding a Room: We need a room approximately 20’ x 20’.
Step 5: Process Consent Forms. The Dream Team will review the forms and contact parents for any needed information or if there will be any cost to them.
Step 6: Hold the Clinic. The Dream Team will arrive promptly to set up and provide preventive services on the date(s) selected.
Step 7: Report Findings. You will receive a report advising the services rendered to the children after our visit.
Step 8: Schedule next clinic.
If you have room in your calendar, we would be happy to schedule another visit during the school year. The American Dental Association recommends repeat applications of the fluoride varnish and we can catch any kids we missed the first time around. These second clinics usually only take one day. |
